One of the main purpose of oral suctioning is to maintain an patent airway, improving oxygenation by removing all the mucous secretions and foreign material from the mouth and throat. The oral suctioning is generally a kind of process that uses a rigid plastic suction known as Yankauer Suction Set, in order to remove the pharyngeal secretions through the mouth.
This suction holder has a large hole for the thumb to cover an initiate suction, followed by the smaller holes in the end where the mucous enters and the suction is applied. Oral suctioning is even today considered to be useful as it helps to clear the secretions from mouth that the patient is unable to remove through effective coughing. Patients who have benefitted the most are the ones who are related to drooling, impaired coughing, and the last but not the least swallowing.
The Safety Considerations
1. Avoid doing oral suctioning on patients who have just had a recent head and neck surgery.
2. Have a clear technique used for oral suctioning.
3. Know who are the patients who are at a risk of suctioning, and are unable to clear all the secretions due to the cough reflex. Have the supplies readily available at the bedside in order to ensure that oral suctioning is required immediately. 4. Avoid stimulating the gag reflex.
5. Try and consider all the other possible causes of the respiratory distress, say for example the pulmonary edema, and the equipment malfunction.
6. If there are any abnormal side effects that occur, and increases the difficulty in breathing then immediately notify about it to the health care provider.
7. Try and avoid the mouth sutures, sensitive tissues and the last but not the least any other tube that is located in the mouth or the nares.
8. Do make sure that you know the appropriate suctioning limits, followed by the risks of applying excessive or inadequate pressure.
Steps before You Begin Dealing with Yankauer Suction Set:
1. Assessing the patient on the need for suctioning, risk for aspirations, inability to protect the own air way, and the last but not the least clearing secretions that may lead you to upper air way obstruction.
2. Speaking to the patient and telling them how the entire process will turn out to be, how it will help in secretions and it will be lasting just for a few seconds. If appropriate encourage them to cough.
3. Asking the patient to position themselves in the semi fowler position with the head being turned on the side.
4. Performing the hand hygiene, gathering the supplies and the last but not the least wearing non sterile gloves. Do not hesitate to wear a mask if there are chances of fluid splash to occur.
5. Have the basin filled with enough water.
6. Have one end of the connection attached to the tubing of the suction machine, followed by the other end to the Yankauer.
7. Then turn on the suction to the required level. Have a test done by covering hole on the Yankauer thumb and suctioning a smaller amount of water.
8. Have the oxygen mask removed if present on the patient. The nasal prongs may be left in place. Have a small towel placed on the chest of the patient.
9. Ensure that the patient is comfortable in the position, and the call bell is within the reach. Don’t not hesitate to provide oral hygiene if necessary and required.
10. Clean up the supplies, remove the gloves, and wash your hands immediately. Have the documents procedure done according to the hospital policy.
